SYNOPSIS:
After Big Infant escapes from the psychiatric facility he is still on large. His presents at the local maternity clinic and his caption on the CCTV camera have risen terror in the Sunshine City. One of the targeted woman Norah Olivine is released after giving birth. Later she and her husband John are found dead in the car. Unlike the past victims her breast are not cut off. This gives confusion among the detectives if it is Big Infant or it is another killer. However, autopsy reviews that Norah was strangled to death while the assailant suck on her breast and John was the first one to die after being struck by a harmer.Homicide Detective Ken and Judy his junior ask for police strong protection at the residents of the released patient.
Another woman Len Brown is discharged and escorted by two police officers because her husband is away. Big Infant attacks the police officers watching over her house, breaks in Len's house and kills her. Another woman Debra is discharged. Judy and Ken escort the woman. The patrol cars roam around Debra's place. The expected Big Infant didn't show up. On the following day Judy and Ken are the ones patrolling around Debra's house. While they are parking near Debra's house their attention is attracted by a drunken woman wavering her car about in the street. They stops the woman. As Ken tries to arrest the drunken lady they heard Debra screaming. Ken and Judy forget about the drunken lady and rushes to check on Debra. They found her alive and she tells them that the killer had showed up and attempts to strangle her. They heard a car raving out. Judy comes out to check and sees a man being picked up by the drunken woman.Suspecting it is Big Infant, Judy gives a chase, but her car swerves out of the road and crushes into a tree.
On the following day, Beulah a friend to Judy delivers a baby. That night Big Infant sneaks into the maternal clinic. He stalks Kim, Beulah's roommate. He kills her in the bathroom and hides the body after breast feeding on her. Big infant continues to kill a male midwife and other staff who comes on his way. Disguising as a midwife Big Infant is called by a nurse to assist a woman in labor. After receiving the baby Big Infant kills both the nurse and the mother. Judy and Ken goes and checks Big Infant at his place. Realizing that the gifts he sends to the women in labor didn't have a tracker Judy becomes alert that Big Infant should have changed his code.
Big Infant enters into nursery He encounters Beulah telling a staff about Kim's missing. Big Infant kills two other ward mates. He goes to the admission and kills the two staff and the guard. He locks the entrances. And cut off the land lines. Beulah enters into the ward with the two corpse seeking Kim. She noticed the corpse and dashes out into the passage and encounters the Big Infant. Beulah escapes the attack from Big Infant after biting his finger. Big Infant chases her.
Beulah discovers more corpse around the hospital both the women and the staff members. Beulah returns to her ward and picks up her infant. She sends a message to Judy and decides to escape from the building before Big Infant kills her. Struggling to walk Beulah goes and hides in the kitchen. Her infant starts to cry exposing her hiding place to the attacker. Big infant enters into the kitchen spot Beulah. Knowing that he won't hurt the infant, she places the infant on safe place planning to run away alone. Big Infant reaches on the tiredness Beulah and starts to strangle her while sucking on her breast. Judy arrives at the hospital. She breaks in and runs around looking for Beulah. She gets in kitchen in time before Beulah pass out. Judy shoots Big Infant at his shoulder and the second shot misses him. Big Infant throws himself out of the building through the window and splashes in the fishing pond.
